Allied Benefit Systems, LLC ("Allied") is committed to keeping your personal information confidential and
secure. This notice describes how personal information about you may be used and disclosed, and how you can get
access to this information. Please review it carefully.
Information Subject to this Notice
This notice applies, in part, to information about you called "protected health information" that is protected
under the federal law "HIPAA," which stands for the Health Insurance Portability and Accountability Act. Protected
health information includes health-related information that Allied collects, creates, receives or maintains in
connection with your Allied user account and that reasonably could be used to identify you. Allied may receive
your health information directly from you or from a third party, such as your employer-sponsored health plan.

We may use and share your protected health information in the several ways without first asking for your written

Other Ways We May Use or Share Health Information
We may and sometimes are required to use or share your health information in special circumstances without first
asking for your written permission. We must meet many conditions in the law before we can share your information
for these purposes.
How We May Use and Share Your Health Information with Individuals Involved in Your Care or Payment for Care
Generally, we will not share your health information or communicate with someone other than you about your user
account.
If you are represented by a legally appointed personal representative, we will communicate with your
representative in the same manner we would communicate with you.
In special circumstances, we may share your health information or communicate with individuals identified as
family, close, friends, or others involved in your care with your permission or, if you are unable to give
permission, only if we believe it is necessary and in your best interest.
To protect your privacy and the security of your user account, we strongly recommend that you never share your
account password or other sign-in information with anyone else. When someone signs into your user account, we
will assume that you gave your permission to that person to access the information in your user account.
Information Collection, Use, and Sharing
If you create an account through Allied's website and/or mobile application service (collectively "site"), you may
be asked to provide personal information, including but not limited to, your name, telephone number, mailing
address, email address, gender, health insurance information, and other such information.
Allied may collect non-personal information about your activity on the site. This information, if collected, may
be collected via computer code sent to your computer (commonly referred to as "cookies" or "web beacons").
"Cookies" are small text files that are placed on your device by a web server when you access our services. We
may
use both session Cookies and persistent Cookies to identify that you've logged into the services and to tell us
how and when you interact with our services. We may also use Cookies to monitor aggregate usage and web traffic
routing on our services and to customize and improve our services. Unlike persistent Cookies, session Cookies
are
deleted when you log off from the services and close your browser. Although most browsers automatically accept
Cookies, you can change your browser options to stop automatically accepting Cookies or to prompt you before
accepting Cookies. Please note, however, that if you don't accept Cookies, you may not be able to access all
portions or features of the services. Some third-party services providers that we engage may also place their
own
Cookies on your device. Please note that this Privacy Policy covers only our use of Cookies and does not include
use of Cookies by such third parties.
"Web Beacons" (also known as Web bugs, pixel tags or clear GIFs) are tiny graphics with a unique identifier that
may be included on our services for several purposes, including to deliver or communicate with Cookies, to track
and measure the performance of our services, to monitor how many visitors view our services, and to monitor the
effectiveness of our advertising. Unlike Cookies, which are stored on the user's hard drive, Web Beacons are
typically embedded invisibly on web pages (or in an e-mail).
The site may automatically record certain information about how our users (each, a "User") use our services (we
refer to this information as "Log Data"). Log Data may include information such as a User's Internet Protocol (IP)
address, browser type, operating system, the web page that a User was visiting before accessing our services,the
pages or features of our services to which a User browsed and the time spent on those pages or features, search
terms, the links on our services that a User clicked on and other statistics. We use Log Data to administer the
services and we analyze (and may engage third parties to analyze) Log Data to improve, customize and enhance our
services by expanding their features and functionality and tailoring them to our Users' needs and preferences.
We may collect your IP address and other information about your online activity to generate aggregate,
non-identifying information about how our services are used and analytics data regarding Users' interactions with
our services.
We collect certain information that your mobile device sends when you use our services. For example, we may
collect a device identifier, user settings and the operating system of your device, as well as information about
your use of our services.
When you use the site, we may collect and store information about your location by converting your IP address into
a rough geo-location or by accessing your mobile device's GPS coordinates if you enable location services on your
device. We may use location information to improve and personalize our services for you. If you do not want us to
collect location information, you may disable that feature on your mobile device.
Allied uses non-personal information to manage the site. Allied may analyze the data about visits to the site to
make it more accessible and interesting for visitors. Further, Allied may share this data with third party service
providers associated with the maintenance of the site. Additionally, Allied may disclose non-personal information
about pages you visit on the site, as well as the frequency with which you visit pages, but not in a manner that
that is inconsistent with the applicable law. We will not sell or rent this information to anyone.
Any personal info submitted to the site will only be used for the purpose requested, for which it is collected, or
authorized. That information may be stored and maintained by Allied. Allied may share this information with
third-party service providers that work with us to administer and provide the services. These third-party service
providers have access to your personal information and financial information only for the purpose of performing
services on our behalf.
We will not share your information with any third party outside of our organization, other than with trusted
partners to help us fulfill your request, perform statistical analysis, send you email or postal mail, provide
customer support, or to provide other services to Allied or site users. Allied has taken and will continue to take
measures to ensure the secure and safe handling of your personal information.
Where Allied shares your personal information with third party providers for the purposes of performing services
related to analytics, reporting, marketing, research, or any other product that may be monetized, your protected
health information will be de-identified in accordance with sections 164.514(b) and(c) of the Health Insurance
Portability and Accountability Act of 1996 (HIPAA) Privacy Rule. De-identification requires the elimination of
primary or obvious identifiers and secondary identifiers through which a user could determine an individual's
identity. Personal information used for research, including public health research, should be de-identified at the
point of data collection for research protocols approved by Allied, unless the individual voluntarily and
expressly consents to the use of his/her personally identifiable information or a waiver of authorization is
obtained.
Allied may disclose any personal or non-personal information collected to the extent it reasonably believes
that
such disclosure is necessary to comply with the law, such as in response to any subpoena, to the extent
reasonably
necessary to establish or defend a legal claim and for other purposes permitted by applicable law.
If you want us to delete your Allied Account, please contact us via email
at websupport-cs@alliedbenefit.com with
your request. We'll take steps to delete your Allied Account as soon as we can.
Additional Information
Where your health plan has contracted with us to provide our services to you, we may forward some of your
requests
to the health plan for response or input as required by our contract.
Our Responsibilities

Allied can change the terms of this notice, and the changes will apply to
