Deck Chair Sign

Papuloerythroderma of Ofuji (PEO) is a rare inflammatory skin disorder characterised by widespread pruritic papular eruptions that coalesce into larger plaques sparing the skin folds. This produces the "deck-chair sign", which is pathognomonic of PEO. The "deck-chair" sign is a sparing phenomenon in which the intertriginous areas and transverse skin folds are spared during an erythematous eruption.

It has been associated with many dermatologic disease states. We report 3 cases of dermatomyositis which presented with the "deck-chair" sign. Biopsies showed interface dermatitis in all.

Deck-chair sign is a unique skin eruption characterized by widespread erythematous papules that become erythrodermic with spare skin folds. An association between the deck-chair sign and malignancies, especially hematological neoplasms, has been suggested. Classically, the "deck-chair" sign is described in papulo-erythroderma of Ofuji, an inflammatory disorder characterized by coalescence of solid papules that typically "spare the skin folds" such as those of abdomen and antecubital and axillary areas.

Deck chair sign (DCS) was first described in papuloerythroderma of Ofuji as the characteristic pattern of sparing of the skin folds which resembles the narrow strips of a deck chair. The sparing of skin folds includes areas such as the inguinal, axillary, sub-mammary, and flexural regions.

Although first described in papuloerythroderma of Ofuji, it is also seen in other inflammatory dermatoses. Papuloerythroderma of Ofuji (PEO) is a rare skin condition first described in 1984 and characterized by diffuse erythroderma composed of papules coalescing into plaques with sparing of skin folds, known as the deck.
