Leaking Bath Tap Mixer Screwfix

Hi All, Got a mixer tap which is constantly dripping from where the tap swivels, also the outlet (pics attached). There's a small grub screw at the back so from what I've seen online I need to remove this, take the tap off and replace the seals, only problem is I've no idea what make this is. Just been trying to fix my father in laws leaky kitchen tap. I managed to get the cross head off the tap - but the cartridge seems to be in embedded in metal/welded in.
