I did all the suggestions I could find online and still nothing. The app displays "Hold to reader" but nothing happens. So I confirmed NFC was working by unlocking and starting the car with Digital Key.

I've seen the Hyundai Digital Key app, but AFAIK it's obsolete now, at least for the 2025 Ioniq 5, which supports Digital Key 2 via Google's Wallet.
That said, my own research has focused on the 2025 Ioniq 5, which I own; it's possible that Hyundai's Digital Key app is still useful for older vehicles. The digital car key lets you use your mobile device to lock, unlock, and start your vehicle, similar to a physical car key. The digital car key requires a connection between your vehicle, mobile device, and Google Account.
Because the connection may not always be available, you should still carry your physical car key in case you need it in order to operate your vehicle.
