It might be possible that the fan left the factory with a motor turning the wrong way. Much more likely is if the fan can't move air out the exhaust, it blows it back out the grill.

The blockage redirected air back down into the bathroom giving the impression that the fan was either installed incorrectly or was otherwise spinning in the wrong direction. Very noisy too. I cleaned it all out as best I could, put it back together and it vents to the outside good, nice and quiet.
