Minecraft community on reddit. The portal frames and achievement were just printed out and stuck on top of cardboard, and the eyes i had to fold by hand. The center was some cool glitter scrapbook paper that i found at the craft store to emulate that portal look, and it's all on top of a black piece of card stock which i then attached to the cap!

Achievements were replaced with advancements around 1.12.2, which were a system Mojang tied better into setup for modding capabilities than the prior. It was a Mojang decision to drop Notch's reference to poorly translated game notifications.
