Important Information About Procedures for Opening a New Account

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

I am applying to Transportation Alliance Bank, Inc. dba TAB Bank, Ogden, UT, the creditor on the Account and issuer of the credit card, through Mission Lane LLC, its servicer, to obtain a credit card account.

Application Agreement

This application is for the Mission Lane Visa® credit card issued by Transportation Alliance Bank, Inc. dba TAB Bank, Ogden, UT.

You certify that all information provided is true, correct, and complete and that you have read the Cardholder Agreement and Credit Card Account Opening Disclosure (collectively the "Agreement"), which may be accessed at www.missionlane.com. You certify that you have the legal capacity to agree to the terms below and in the Agreement. You authorize us, Mission Lane LLC on behalf of TAB Bank, to verify or check any of the information you provided and to obtain credit bureau reports on you that we will use when considering your application for credit. To approve or decline this application, service your account, or manage our relationship with you, you authorize us to obtain information from others to investigate your credit, employment, income history, and state records, including state employment security agency records, and to report information regarding your account to consumer reporting agencies. You acknowledge that we will determine the amount of credit extended based on our review criteria. If you do not qualify for the product or pricing requested or for the lowest pricing offered, you authorize us to offer you the product and pricing for which you do qualify. You agree to be bound by the terms and conditions of the Agreement and this Application Agreement, and understand that the terms of your account may be changed at any time, subject to applicable law. You acknowledge that the rights under the Agreement will be subject to the terms of the Arbitration Agreement described in the Agreement. You understand and agree that you will be liable for payment of all amounts owing on the account.

The minimum credit limit, provided you meet these terms and conditions, is $300. A consumer report from consumer reporting agencies will be obtained in considering this application and for the purposes of any update, renewal, extension of credit, review, or collection of the account. Upon request, we will inform you of the name and address of each consumer reporting agency from which we obtain a consumer report relating to you. If you are a married applicant, you may apply for credit in your own name.

State Laws Require the Following Notices:

California Residents: A married applicant may apply for a separate account. After approval, each applicant shall have the right to use this account to the extent of the credit limit set by the creditor and each applicant may be liable for the amount extended under this account to any joint applicant.

Delaware Residents: Service charges not in excess of those permitted by law will be charged on the outstanding balances from month to month.

New York Residents: New York residents may contact the New York State Department of Financial Services at 1-800-342-3736 to obtain a comparative listing of credit card rates, fees and grace periods.

Ohio Residents: The Ohio laws against discrimination require that all creditors make credit equally available to all creditworthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio Civil Rights Commission administers compliance with this law.

Utah Residents: As required by Utah law, you are hereby notified that a negative credit report reflecting on your credit report may be submitted to a credit reporting agency if you fail to fulfill the terms of your credit obligations.

VT Residents: We will not share information we collect about Vermont residents with companies outside of Mission Lane LLC except as permitted by law or the Vermont resident consents. For information on how the issuing bank manages your personal information, please refer to the bank's privacy notice or the applicable bank website.

Wisconsin Residents: Your signature confirms that this loan obligation is being incurred in the interest of your marriage or family. No provision of any marital property agreement, unilateral statement or court decree adversely affects a creditor's interest unless, prior to the time the credit is granted, the creditor is furnished a copy of the agreement, statement or decree or has actual knowledge of the adverse provision.
