Expand/CollapseHISTORICAL NOTE

The Community Health Services Act of 1976 [Laws 1976 c9] provided block grants from the state to local governments to develop and maintain an integrated system of community health services under local administration with a system of state guidelines and standards. Application could be made by jurisdictions with an aggregate population of 30,000 or more persons in a county or multi-county area (population size was waived if three or more counties combined) or municipalities located in a county with a population of 300,000 or more or any city located in three counties. Each applicant had to have a board/department of health which was responsible for following the act's standards and a local community health services advisory committee.

Expand/CollapseSCOPE AND CONTENTS

The published records and reports include guidelines and criteria created by the office to be used by applicants (1977-1979).

The local annual reports and plans (1977-1983) include community health service written plans submitted by the applicants detailing development, implementation, coordination, and operation of community health services that would meet the priority needs of the community. An application accompanied the plan. Each applicant also had to submit an annual report of the activities of the board of health. The reports and plans were actually produced between 1977 and 1983 but detail both actual and projected (through 1985) activities. There are also local annual reports only for 1989 and 1991.

The local activity reports (1984) are a uniform reporting system developed and implemented by the office; they include statistics on staffing, interaction with community organizations, client visits, client age, clinic visits, payment sources, school health services, emergency medical service activities, health education activities, disease outbreaks and investigations, environmental health activities, food establishment inspections, private well water quality, and home health agency visits.
