Expand/CollapseSCOPE AND CONTENTS

Each certificate includes some or all of the following information: date and place of death; deceased person's name, sex, color or race, marital status, date and place of birth, age, occupation, usual place of residence, length of residence in the United States if foreign born, country of citizenship, social security number, and military service status; spouse's name and number of children; parents' names and birthplaces; cause(s) of death; date and place of burial, cremation, or removal; and date the certificate was filed. Many of the certificates are for Native Americans.

For the most part, these deaths are not documented in the separately cataloged Health Department's death certificates series. There are some, however, especially those for the years following 1915, which were also filed with those certificates.
