The records, in several different formats, generally giving the following information: name, application number, admission number, family and clinical/etiological histories, and remarks. The registers of applications and admissions also often contain individual inmate's photographs.

Register of applications, most of which were accepted and resulted in admission; however, from 1895 on, most were postponed and admitted up to three years after application. Entries contain information on the applicant's general history, physical condition, and clinical history (taken from the applications); assessment of industrial abilities and deportment; remarks relating to discharge, death, or withdrawal of application; name of person who furnished clothing; bond or certificate number; name and address of correspondent; and often a photograph.

Bound applications for admission with personal descriptions (prepared by parents or guardians) and etiological histories (prepared by physicians). Information may include general description and history (relative to locomotion and coordination, speech, mental and sensory characteristics, and mental and industrial "acquirements" among other things); physical condition and clinical history; family history (birthplaces of father and mother, occupation of father, character and condition of brothers and sisters, use of drugs by parents); circumstances of mother's pregnancy, labor, and delivery; and physician's assessment of cause. The applications and etiological histories were sometimes bound together and sometimes separately.
