SCOPE AND CONTENTS
The physician's diary (1894) is a record kept by Dr. John R. Bailey of his daily activities, with notes on patients' illnesses and treatment, including drugs prescribed; the physician's reports are monthly reports on the number of cases and types of diseases treated in each of the institution's departments.

The later histories (1890s-1900) include application number, name, and date of evaluation (?); anthropometrics; evaluations of sensation, perception, emotion, instinct, volition, association, attention, judgment, reasoning, imagination, and special talents; and school record. |
