Information includes name and case file number, results of initial physical examination and mental evaluation, family mental history, hospital record (date, diagnosis, treatment, date discharged), pharmacy prescription record (date, diagnosis, drug or other prescription), and brief periodic assessments of physical and mental status. The records cover admission through 1923, although individual record entries may continue into the 1930s.
