Update on this - the clutch plate was very worn, kinda just in time replacing it, and I believe the low engage was due to worn pressure plate spring. Kinda makes sense if you think about it. I'm replacing all the wear items on front and back suspension (including the subframe) while it's on the lift.

However, an engagement point just off the floor, in conjunction with difficulty placing in gear/notch shifting are clear indicators of a dead clutch. I am surprised it isn't slipping under heavy load in top gear. A one-two shift should always be a slow motion gear change with these 'boxes', IMHO.
