Brainstorming Causes

Once the problem is clearly defined, the team can begin to brainstorm potential causes. These should be listed along the spine of the fish, with major causes acting as branches and sub-causes as sub-branches.
To ensure that all potential causes are considered, it's helpful to use a structured approach. One common method is to use the 5 Whys technique, where the team asks "why" repeatedly until they reach the root cause. Each "why" can be a cause in the Fishbone Diagram.

For example, if one of the causes in the Fishbone Diagram is "late delivery by suppliers", the team might investigate this by asking suppliers why they are delivering late. If the suppliers say that they are delivering on time but the customer's receiving department is not accepting the deliveries, then "late delivery by suppliers" is not the root cause. The team can eliminate this cause from their investigation and move on to the next one.

Once the team has identified a potential root cause, they should verify that it is indeed the root cause of the problem. This can be done through testing or by gathering more data.
For example, if the team has identified "inadequate training of delivery drivers" as the root cause of delivery delays, they might test this by providing additional training to the drivers and seeing if the number of delivery delays decreases.

Once the root cause has been verified, the team can develop and implement solutions to address it. These solutions should be specific, measurable, achievable, relevant, and time-bound (SMART).
For example, if the root cause of delivery delays is "inadequate training of delivery drivers", a SMART solution might be: "By the end of this month, we will provide additional training to all delivery drivers to ensure that they understand the importance of punctuality and the consequences of late deliveries".
