If you've recently had an eye exam and found yourself staring at a seemingly cryptic prescription chart, you're not alone. Understanding a glasses prescription might seem daunting at first, but it's actually quite straightforward once you understand the basics.

Our eyes are complex organs, and a comprehensive eye exam helps diagnose any vision problems and prescribes corrective lenses, if necessary. The prescription chart, or refraction, is a crucial part of this process, providing vital information about your eyes' focus and alignment.

Understanding Your Spherical Power
The spherical power, measured in diopters, is the basic correction for nearsightedness (myopia) or farsightedness (hyperopia). If you have perfect vision, this value is 0.

In myopia (-) values indicate a higher level of nearsightedness, meaning you can see objects clearly up close but have trouble with distant vision. Conversely, hyperopia (+) indicates a higher level of farsightedness, where you can see distant objects clearly, but nearby ones are blurry.
Spherical Power for Nearsightedness

If your spherical power is a negative value, it represents the degree of nearsightedness. For instance, -1.00 indicates mild myopia, while -4.00 denotes a relatively higher level of nearsightedness.
For example, if your prescription reads "-2.50 -0.50 x 180", the "-2.50" part is the spherical power, indicating a moderate level of nearsightedness in both eyes.
Spherical Power for Farsightedness

If your spherical power is a positive value, it signifies a degree of farsightedness. A "+0.50" prescription may not require correction, while a "+3.00" would typically necessitate reading glasses.
In a prescription like "+1.50 +1.00 x 090", the "+1.50" is the spherical power, indicating a relatively low level of farsightedness in each eye.
Cylindrical Power and Cylindrical Axis

Astigmatism, a common vision condition, causes blurred vision due to an irregularly shaped cornea. A cylindrical power corrects this issue by flattering the lens to a more spherical shape.
The cylindrical axis, usually expressed in degrees (0 to 180), determines where the cylinder power is placed to effectively counter the astigmatism. The combination of cylindrical power and axis is called a cylinder value.








Cylindrical Power
The cylindrical power is denoted by the second number in your prescription, preceded by a "+" or "-". A positive value indicates a degree of farsighted astigmatism, while a negative value signified nearsighted astigmatism.
In a prescription like "-2.50 -0.75 x 165", the "-0.75" represents the cylindrical power, working together with the spherical power to correct nearsightedness and astigmatism.
Cylindrical Axis
The cylindrical axis, given in degrees, mainly affects the orientation of the astigmatism correction. In a prescription, it follows the cylindrical power, indicating where the cylinder power should be placed to maximize its effectiveness.
For instance, in the previous prescription "-2.50 -0.75 x 165", the "165" is the cylindrical axis, pointing to the correct direction to place the cylindrical power for optimal refraction.
Pupillary Distance and Add Power
Pupillary distance (PD) measures the distance between the centers of your pupils. This measurement is critical for determining where lenses should be placed in your frames. Add power is an additional prescription for multifocal glasses.
Unfortunately, PD measurement doesn't always appear on standard prescriptions, so you might need to measure it yourself or ask your optician.
Pupillary Distance Measurement
Measure PD by sitting a friend facing you, about 16 inches apart. Using a ruler, align it with the bridge of your nose, then measure from the center of one pupil to the center of the other. If your pupils aren't level, measure from the center of your right pupil to the center of your left pupil.
For children, use this method without sitting them directly facing you. Instead, have them looking at an interesting object or image while you or a helper measures the PD.
Add Power for Multifocal Lenses
Add power is an additional prescription used in multifocal lenses (bifocals, progressives) to correct nearsightedness (myopia) in combination with farsightedness (hyperopia).
It typically ranges from 1.00 to 2.50, with higher values providing more power for near focus. When a prescription includes an "add", it usually follows the spherical power, like in this example: "+1.50 +1.00 add".
