There is one card for each family, giving address; name, age, sex, relation to the family, birthplace, race or nationality, number of years in the United States, and physical defects of each family member; church(es) and lodge(s) belonged to; and newspapers subscribed to. For each person over age 16, the following additional data are given: military service, citizenship status, whether speaks and writes English, other language(s), marital status, birthplace of parents, name of school where attending English classes, whether has a library card, occupation, and whether currently employed.

For each person under age 16, the following additional data are given: name of last or current school attended and grade attained, language(s) besides English, whether working, and if so, how long employed, whether has a work permit, and name of employer.
