Lactobacillus reuteri and Oral Health: What the Research Shows
A specific L. reuteri strain pair is among the most studied oral probiotics for gums. Here's what its trials show, negative results included, and where it doesn't help.

One of the better-studied oral-probiotic approaches for gums uses a specific Lactobacillus reuteri pair. It sits behind a lot of “gum health” marketing, ProDentim included. But the evidence belongs to the named strains, delivery format and treatment setting used in the trials, not to every product that prints the species name.

What L. reuteri is
The studied version isn’t just “L. reuteri.” It’s a specific commercial pairing of two strains, DSM 17938 + ATCC PTA 5289 (sold as “Prodentis”). That strain code matters enormously. With probiotics, the evidence attaches to the exact strain, not the species, so a label that says only “Lactobacillus reuteri” can’t automatically claim the research below. Always look for the code.

What the trials show for gum health
The consistent, honest finding: L. reuteri helps most as an add-on to a dentist’s deep cleaning (scaling and root planing) in periodontitis, the more serious form of gum disease.
- Two well-run trials found that taking L. reuteri lozenges alongside a professional cleaning produced extra improvement in pocket depth and gum attachment, with benefits tracked out to a year (PMID 24164569; PMID 25741580).
- A meta-analysis of eleven trials agreed there’s a benefit as an add-on to cleaning, but described it as modest and short-term (PMID 32373987).
So far, so good, with two honest words attached: adjunct and short-term.
Where it doesn’t help
Now the results the sales pages bury.
- Plain gingivitis on its own. A meta-analysis found the L. reuteri effect on gum and plaque scores was not statistically significant versus placebo (PMID 31682012). That’s the exact scenario most buyers are in: mild gum issues, no professional cleaning. In that setting, the strongest evidence says it doesn’t clearly beat a placebo.
- Smokers. A trial in smokers found no significant difference between the probiotic and placebo (PMID 30931588). The effect isn’t universal.
None of this means L. reuteri is a scam. It means the honest claim is narrow: a studied add-on to professional gum treatment, not a standalone cure. NIDCR describes periodontal treatment as infection control tailored to disease extent and notes that a dentist may refer a patient to a periodontist; a probiotic does not replace that assessment or treatment (NIDCR, reviewed November 2024). It also has no use for regrowing gum that has already receded (that’s surgical, covered on the receding gums page).
Breath and other endpoints
L. reuteri is studied mostly for gums, not breath, so if fresh breath is your goal, a strain like BLIS K12 has the more direct case (see the K12 and M18 page). There’s also early interest in L. reuteri and Candida, the yeast behind oral thrush, but that evidence is preliminary and population-specific, covered on the oral thrush page.
Frequently asked questions
Does L. reuteri cure gum disease?
No. It may support the results of a professional cleaning short-term (PMID 32373987). It doesn’t replace the cleaning, and it doesn’t reverse bone or tissue loss.
Will it help my mild gingivitis if I just take it?
The best evidence says probably not on its own (PMID 31682012). Hygiene, and a cleaning if you’re overdue, do the real work.
Does the exact strain matter?
Yes, hugely. The studied pair is DSM 17938 + ATCC PTA 5289. A product that doesn’t print a strain code can’t claim this research. Compare labels in the oral probiotics guide.
Is it safe?
The studied strain pair was generally well tolerated in the cited trials, but that does not establish blanket safety for every product or patient. If you’re immunocompromised, seriously ill, pregnant or considering it for a child, ask a clinician first.
How to check a label in 30 seconds
Find the letters and numbers after L. reuteri. The gum studies most often discussed here used DSM 17938 together with ATCC PTA 5289. A label that says only “Lactobacillus reuteri” does not prove it contains either strain. Next, check whether the product dissolves in the mouth or is swallowed immediately; direct oral contact is closer to the studied lozenge format.
Then check the claim. Trials tested the strain pair as an add-on to cleaning or periodontal treatment, not as a way to replace either. A seller that jumps from those studies to “rebuilds gums” or “reverses cavities” has moved beyond the evidence.
When a trial is worth judging
Set one modest outcome before starting, such as morning breath or gum comfort, and keep brushing and flossing unchanged. Reassess within the product’s return window. Stop and seek care if symptoms worsen; continuing a supplement is not a substitute for examining persistent bleeding or pain.
